Membership Information and Forms

Membership in the Society is open to all.  Membership dues include a subscription to our journal.  Dues rates are: $12.50 US per year for addresses in the United States (including Zip Codes in Puerto Rico), Canada and Mexico; $15.00 US per year elsewhere; and a special rate of $10.00 US per year for those who choose to receive our journal in *.pdf format by e-mail.  Use the links below to select a membership application form to suit your needs: PDF to print a hard copy, fill out and mail in traditional style; FORM to download an MSWord Form version of the application, which you may complete on your computer.  Payment must accompany your application.

We regret that at this time, we are no longer able to accept or process Pay Pal payment of dues.  Pay Pal has raised objections and demanded information regarding our nonprofit status which goes beyond the requirements of the Internal Revenue Service when we obtained nonprofit status in the late 1970's.  We are not prepared to submit the Society to their nongovernmental review of our status, and therefore have ceased to use their services.
